LPN
HOURS: Monday-Thursday, 8am-5pm and Friday 8am-12pm
Job overview: Provides direct and indirect patient care in the office/clinic setting.
RESPONSIBILITIES:
• Provides direct patient care under the supervision of a RN and/or physician, evaluates outcomes, consults with other health team members as required and adjusts nursing care processes as indicated to ensure optimal patient care.
• Ability to perform a head-to-toe assessment, under the supervision of a RN and/or physician, on all patients as per policy. This includes neonate, pediatric, adolescent and geriatric patients and the general patient population.
• Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
• Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
• Performs patient care responsibilities considering needs specific to the standard of care for patient's age.
• Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to the patients served. Able to interpret data about the patient’s status in order to identify each patient’s age specific needs and provide care needed by the patient group.
• Maintains current knowledge of medications and their correct administration based on age of the patient and his/her clinical condition.
• Follows the five medication rights and reduces the potential for medication errors.
• Medical and surgical asepsis is carried out during treatments and special procedures.
• Communicates appropriately and clearly to coworkers, physicians and office/clinic manager.
• Demonstrates the ability to assist physicians with procedures and perform services requiring technical and manual skills under the direction of a RN and/or physician.
• Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and family.
• Interacts professionally with patient and family and involves patient and family in patient education.
• Formulates a teaching plan based on identified learning needs and evaluates effectiveness of learning; includes family in teaching, as appropriate.
• Demonstrates ability to revise plan of care as indicated by the patient's response to treatment.
• Demonstrates the ability to perform treatments and provide services to level of licensure.
• Meets current documentation standards and policies.
• Demonstrates knowledge of lab requirements, requisitions.
• Maintains established office/clinic policies and procedures, objectives, performance improvement program, safety, environmental and infection control standards.
• Maintains a safe, comfortable and therapeutic environment for patients and families in accordance with office/clinic standards.
• Answers telephone calls in a professional manner; refers patient problems to office/clinic physician; schedules patient appointments.
• Ensures an adequate stock of supplies.
• Assists in cost containment through appropriate ordering and conserving of supplies and equipment.
• Maintains a good working relationship with all staff members of the office/clinic.
• Manages and operates equipment safely and correctly. Performs preventative maintenance.
• Demonstrates the ability to be flexible, organized and function under stressful situations.
• Response is appropriate to the patient in an emergency or physically distressful situations.
• Performs other duties as assigned within the LPN/LVN scope of practice.
EDUCATION:
• Current LVN/LPN licensure in the State of Georgia.
EXPERIENCE:
• Professional nursing experience in a clinical setting required.
• Experience working in a Rural Health Clinic is a plus.
