RN (Nights) FT
HOURS: Full-time, Night shift, 7pm-7am. Some weekend work required.

RESPONSIBILITIES:

• Performs admission and ongoing physical-psycho-social assessments on assigned patients utilizing all available resources (Ex. Physical examination, patient, significant others, medical records, other health care professionals, etc.) and documents assessment in the medical record.
• Formulates and documents a patient problem list/nursing diagnosis including physical, psycho-social, spiritual, learning, safety, and infection control needs.
• Identifies expected outcomes and nursing interventions based on all available assessment data.
• Develops a multi-disciplinary plan of care based on nursing and medical diagnosis, nursing standards of care/practice, and patient needs (including growth and development) in collaboration with patient, significant others, and other members of the health care team and modifies plan as required by changing patient needs.
• Develops patient discharge plan utilizing available resources (Ex. Patient, significant other, social services, physicians, education department, etc.).
• Performs technical aspects of patient according to established policies and procedures; adapts techniques to growth and development stage of patient and unique patient needs.
• Provides education for patient and significant others (Ex. Explanation of procedures, therapeutic and adverse effects of medications, drug and food interactions, and discharge plans) utilizing available teaching resources and documents/significant other’s understanding in medical record.
• Recognizes obvious and subtle changes in patient’s physical and mental status as well as abnormal test results and responds appropriately.
• Demonstrates the ability to initiate, and direct life support measures and perform effectively in Code Pink procedures.
• Demonstrates accountability for administration, observation, documentation, and reporting effects of drugs, intravenous procedures, other diagnostic and therapeutic measures in an accurate and safe manner.
• Demonstrates responsibility for proper use and safe keeping of all controlled medications kept on the nursing unit and monitors the administration and documentation of use.
• Transcribes and/or counter signs physician orders; receives physician orders for assigned patients.
• Documents nursing interventions and an evaluation of their effectiveness to include the patient’s response to therapy and the progress toward the attainment of care goals in the patient’s medical record in accordance with the established procedures as outlined in Dodge County Hospital’s policy and procedure manual and communicates this information to other members of the health care team.
• Maintains the dignity, rights, and confidentiality of the patient; serves as a patient advocate at all times.
• Participates in the evaluation of the quality of patient care and continuous quality improvement activities.
• Participates in hospital emergency, fire, and disaster drills; knows personal role in case of a fire and demonstrates the ability to direct the activities of the nursing unit in the event of an emergency situation or patient evacuation.
• Follows hospital and departmental policies and procedures including but not limited to safety and infection control.
• Acts as a preceptor for new employees as assigned.
• Assumes the responsibility of the charge nurse/team leader as designated: Supervises up to 20 other RN’s, LPN’s, Nursing Assistants, Unit Clerks, Student Nurses, and other members of the health care team for assigned patients for assigned shifts. Assesses the staffing needs of the unit to provide quality patient care on the unit and collaborates with the nurse manager or supervisor in adjusting staffing levels and individual assignments based on staff and needs of patient. Checks emergency crash cart and stocks drugs; ensures that pre-established level of emergency supplies is on hand at all times.
• Adheres to dress code. Wears identification while on duty.
• Attends committee meetings as appropriate.
• Reports to work on time and as scheduled.
• Performs other duties as assigned.

EDUCATION:
BLS Certification preferred
RN Certification required
