Patient Rights & Responsibilities

Dodge County Hospital respects the rights of each patient, recognizes that each patient is an individual with unique healthcare needs, respects personal dignity, and provides considerate, respectful care on individual needs. Dodge County Hospital supports your right to make decisions regarding your medical care, including the decision to discontinue treatment to the extent permitted by law. Dodge County Hospital will assist you in the exercise of your rights and will inform you of any responsibilities related to the exercise of those rights.

Your Rights:

  • Receive treatment regardless of disabilities, creed, gender, national origin, age, sexual orientation or sources of payment
  • Considerate, respectful care that respects your psychosocial, spiritual and cultural values
    Personal privacy
  • Confidentiality of personal information
  • A safe environment
  • Know the names and professional status of your caregivers
  • Complete and accurate information concerning your health
  • To be informed and make decisions about your care including:
    • Information about any proposed procedures, including any risks, expected or known possible outcomes, reasonable alternative treatment options.
    • Participation in choosing your treatment or consulting with a specialist.
      Expecting your advance directive to be honored.
    • Refusing care and treatment within the limits of law and regulation
    • Including or excluding family in care
    • Effective communication. This includes communication assistance for patients with special needs and assistance in communication with family members or significant others outside the hospital and access to protective and advocacy services.
  • An itemized explanation of your hospital bill
  • Timely resolution of any complaints regarding your care
  • Management of pain

Your Responsibilities

  • Providing caregivers with complete and accurate health information both present and past complaints and illnesses
  • Letting your caregivers know whether you clearly understand the planned treatment and what is expected of you
  • Following the treatment plan and keeping appointments or canceling in advance
  • Your actions if you refuse treatment or do not follow the healthcare provider’s instructions
  • Making sure the financial obligations of your healthcare are fulfilled as soon as possible
  • Following hospital rules and regulations affecting the patient care and conduct, including not smoking
  • Being considerate of the rights and property of others
